Home Care Packages are a combination of services subsidised by the government, to help eligible older adults continue to live safely and independently at home. Services are based on individual needs, and people who have a Home Care Package have the choice and flexibility over who delivers care, and the way support is provided. To see if you’re eligible for a Home Care Package, you must first be assessed by an Aged Care Assessment Team (ACAT), who will identify the right services and level of care for your needs. The assessment will determine which level of care applies for you, from Level 1 for low care to Level 4 for high care. The higher the level, the more funding the government pays on your behalf, allowing you to buy more services. If you are assigned a Home Care Package, you must choose an approved provider to host your package on your behalf, to coordinate the care and supports you need. For more information about accessing aged care services, or to arrange an ACAT assessment, please call My Aged Care on 1800 200 422 or visit www.myagedcare.gov.au. Carers Link – An Approved Home Care Package Provider As an approved Home Care Package provider, Carers Link will work in partnership with you to tailor care and services to support your needs and goals.
