Transition Care

Transition Care is a package of short-term care provided for up to 12 weeks, to help older people who need help to return home after a hospital stay.

Different services may include physiotherapy, occupational therapy, nursing support or personal care for example, with the goal of optimising the person’s functioning and independence.

Carers Link is an approved Transition Care provider. We provide assistance with personal care, domestic duties, medication prompting, meal preparation, shopping and transport to appointments.
